"Investigative accounts concerning vampires were documented throughout the medieval Europe and had to be sealed by either a local official or doctor. These are reproductions of actual seals found on these reports."

Here is a handy equipment list of my own making in both Microsoft Works and Excel files.

SITE REPORT

For some of the fields that I have on the form, here are some explanations of the terms plus suggested labels and ideas.

Location Type : Residence, Business, Hotel, Cemetery, Tavern, Theatre, School, Hospital, Sanitarium, Prison

Purpose: Original purpose of structure. Was it first a residence and then converted to a motel? Or a mortuary into a restaurant?

Type of Activity Reported: Apparition, Phantom, Poltergeist, Sounds, Voices, Shadows, Presence, Cold Spots, Anomalous Electrical Problems, Nightmares, Odors

Investigation Category:  More than one can apply.

RECON: Initial visit by the prime members or the contact for the group to get preliminary data for a possible follow up field investigation. Includes a walk-thru, mapping or general layout of the area, some cursory EMF, temp readings and still photos. Brief interview with staff/residents. Done when the resources required for a full field investigation are uncertain.

PRIMARY FIELD: Full scale investigation with several members and all available equipment. Preferably there are multiple walk-thrus and more detailed sensor/psychometric information along with photo/video documentation.  Manresa and Seattle Underground were good examples of this.

CEMETERY RUN / GRAVEYARD SHIFT: Same as above but in a burial ground.

PRIMARY FIELD, LIGHT: As primary field, but number of team members limited due to nature and size of the investigation. Residential investigations may fall into this category.

PRIMARY FIELD, FOLLOW-UP: Repeat visit to a site because of indeterminate or promising results.

VIGIL: Site has shown enough promise in a previous investigation to stake out particular areas overnight or for an extended period of time. Characterized by camera setups, EMF/sensor array deployment, EVP traps.

SPEC OPS: This particular investigation has peculiar characteristics which places constraints besides just the number of investigators involved, such as requiring special equipment or an unusual procedure. An example would be an outdoor spot that requires hiking/camping gear. Or a place where tech equipment is unreliable, doesn't work and requires a team composed mostly of sensitives.

LIAISON: A small team developed for communicating the results of an investigation to a client, follow-up counseling, or for negotiating future hunts.

EQUIPMENT TESTING: An investigation whose primary objective is to test a new procedure or piece of gear.

CLASSIFIED: - Information on this investigation is restricted - see notes

PROBABLE HAUNTING TYPE:  See CLASSIFICATION section for more details. Here are probability ratings for these hauntings:

MINIMAL - Haunting in the classic sense is very unlikely. No evidence whatsoever, and it is highly likely or confirmed that the haunting is fictional due to other factors. (someone admits making up the legend for financial gain, mental illness, etc.)

LOW - Some chance of a classic haunting, but no evidence to support it found at the site. Witness testimony inconsistent or too likely natural causes are at work.

MODERATE - No hard evidence found, but historical accounts are consistent enough to warrant further investigation, perhaps a vigil. Suggestion of some other type of activity at work besides a classic haunting, such as PK or spiritual residue.

PROBABLE -  Unexplained events are witnessed by one or more investigators and there is historical evidence to support a particular type of haunting.

CONFIRMED - Evidence from a number of sources and witnesses overwhelmingly points to a classic haunting of some sort.

MANIFESTATION LEVELS

This was created by a fellow parapsychologist. You can find his site at:

I give Ray full credit here.

Manifestation Level One - L1
Primary Determinant- Very subtle "suggestive
indications of a possible ghost presence.
Examples: Feeling being watched, seeing movement in the corner of the eye, hearing of voices by a single observer and "cold feeling" areas.

Manifestation Level Two - L2
Primary Determinant- Direction of Phenomenon by
instruments
Examples: Consists of manifestations detectable with a temperature and or EMF meter. L2 ghosts are invisible, but sometimes may leave images on film. Most EVP (Electronic Voice Phenomena) happens here. This is the most common verifiable level.

Manifestation Level Three - L3
Primary Determinant- Direct Observation of sounds or
imagery of an "objective nature."
Includes VISUAL and/or AUDIO (words) phenomena having been witnessed by more than one person. (Not necessary at one time.) The are detected by the normal senses without aid.

Manifestation Level Four - L4
Primary Determinant- Physical Effects
Includes the movement of objects, whether by direct observation or being found in a new place when no other forces could be involved.

Manifestation Level Five - L5
Primary Determinant- Well formed, interactive
apparition.
Will range from slightly transparent to absolutely solid entity. Rare, but they do exist. In some cases, the ghost aspect is not apparent until they suddenly "vanish."

Ray: "Personally, I like the University. They gave us money and facilities; we didn't have to produce anything. You've never been out of college. You don't know what it's like out there. I've worked in the private sector. They expect results."
